HIPAA Release

 

So that your health care provider is not bound by law to withhold your confidential health care information from anyone but you when you are unable to make your own medical decisions, we are happy to prepare a HIPAA Release for you. If you ever become physically or mentally ill and unable to make your own medical decisions, your trusted agent will need access to your protected health care information to make the best decisions for you.

 

A HIPAA Release allows your doctors, other health care providers, and insurance company to discuss your confidential health care information with your agent for treatment, billing, and Out-of-Hospital Do Not Resuscitate Information and Form (DNR). 

 

Out-of-Hospital DNR (Do Not Resuscitate)

 

So that you may pass on naturally with peace and dignity if it is not your desire to receive artificial resuscitation, we are happy to help you prepare an Out-of-Hospital DNR. The Out-of-Hospital DNR instructs emergency medical personnel and other health care providers not to resuscitate you.
